Definitely agree that a real surface reading of this movie could lead one to thinking "boy that's exploitative" and also that actually engaging with it shows it to be really deeply felt and somehow very emotionally true.
Another film that very much lives in this world (without the addition of the supernatural) is The Father. No movie I've seen better conveys with what it's like to be inside that affected brain than that one.
